How to submit your booster verification information

All PLU students, faculty, and staff will now be required to get a booster shot by Monday, February 7, 2022, or within 14 days of their eligibility. Visit the State Department of Health (DOH) website to determine your timeline for eligibility.

3 Steps to Upload Vaccination Verification

  1. Go to our Etrieve Portal website
    1. Student Form
    2. Employee Form
  2. Enter your PLU ePass login username and password
  3. Follow the steps to indicate vaccination booster status and upload your documents (please note that verification takes approximately ten business days).

If you are a student employee you only have to submit your material through the student form.

Students: If you have already submitted your initial COVID-19 vaccination material, you are only required to submit your booster information.

Employees: Even if you have already submitted to Human Resources, we would like you to resubmit the information.

If you feel that you are ineligible for the booster because of severe side effects from the initial series, please contact your healthcare provider to request a medical exemption using the PLU Medical Exemption form (students) or the Vaccine Exemption Request form (staff/faculty).

If you have received your vaccination in Washington state you can obtain a digital COVID-19 vaccination record through the Washington State Health Department verification website.

Health Department Vaccine Clinics at PLU

The Tacoma Pierce County Health Department will be running COVID Vaccine Clinics on the PLU campus through the end of February. The clinics are open to the public.

Clinic Schedule: through February

The Moderna, Pfizer and Johnson & Johnson vaccines will be available for 1st shots, 2nd shots and boosters.

The clinics are walk-in only, no pre-registration required, except for children ages 5-11 years old for the Pfizer vaccine.
